In accordance with California Government Code § 11015.5 and the Information Practices Act of 1977 (Civil Code § 1798 et seq.), and as required by the Statewide Information Management Manual (SIMM) 5310-A, the California Department of Technology (CDT) provides the following notice regarding the collection and use of personal information when completing the Contact Us form (online or hardcopy). Personal information may include: individual’s name, social security number, physical description, home address, home telephone number, education, financial matters, medical or employment history.

Purpose of the Collection

CDT collects the personal information you voluntarily provide to carry out its statutory functions and responsibilities, including but not limited to:

  • Approval and oversight of information technology projects
  • Oversight of statewide IT strategic planning, project delivery, procurement, policy and standards, and enterprise architecture
  • Providing public services
  • Conducting outreach or research
  • Complying with legal obligations

Authority for Collection

The collection and use of your personal information is authorized under:

  • Government Code sections 11545-11548
  • Government Code sections 11015.5 and 11019.9
  • Information Practices Act – Civil Code section 1798 et seq.
  • Public Records Act – Government Code section 7920 et seq.
  • Statewide Information Management Manual (SIMM) 5310 – A & B
  • Applicable state or federal program requirements

Use of Information

The personal information you provide may be used for purposes including, but not limited to:

  • Verifying your identity
  • Responding to your requests
  • Processing transactions or applications
  • Ensuring program integrity and compliance
  • Communicating with you about services or benefits

Disclosure of Information

Personal information may be shared only as authorized by law, including with:

  • Other government agencies for verification and program administration
  • Contractors or vendors under strict confidentiality agreements
  • Law enforcement or regulatory agencies as required by law

Consequences of Not Providing Information

Providing personal information is voluntary. However, failure to provide required information may result in the inability to process your request or provide certain services. Please do not include any personal information beyond what is specifically requested in this Contact Us inquiry form.
